We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.

The dynamic nature of home care service places emphasis and demands on the experience and expertise of the LDR staff and management. We carry high levels of responsibility and our reliability is associated with all  aspects of our work. We have a commitment to continued professional development and training to ensure that we are all suitably qualified and equipped to meet these expectations.

245D Basic Support Services

Provides the level of assistance, supervision, and care necessary to ensure the health and welfare of the person and does not include services specifically directed toward the person's training,  treatment, habilitation, or rehabilitation. 

  • In-home respite care
  • Individualized Home Support w/o training
  • Homemaker (cleaning/ with personal care assist/ home management)
  • Individual Community Living Supports

245D Intensive Support Services

Provides assistance, supervision, and care that is necessary to ensure the health and welfare of the person and services specifically directed toward the training, habilitation, or rehabilitation of the person.  

  • Individualized Home Supports
  • Semi-independent Living Services
  • Supported Living Services

DHS License #1081915

Medical Assistance (MA)

(MA) is a health insurance program for low-income people of any age. Apply through your local county human services department. For more information, contact the Senior LinkAge Line at 1-800-333-2433, or the Disability LinkAge Line at 1-866-333-2466. 


DHS contracts with managed care organizations (MCOs) to provide health care services for MHCP. Currently serving: 

  • Blue Plus
  • South Country Health Alliance (SCHA)
  • UCare (currently under accreditation- check back)

Medical Assistance Home and Community-Based Services Programs:

Alternative Care Program

Is a program offering certain home and community-based services for Minnesotans at least age 65 years, at-risk of nursing home placement, and with low income and assets. 

Brain Injury Waiver

Is for people with a traumatic or acquired brain injury, and who need the level of care provided in a nursing facility or neurobehavioral hospital. This program offers specialized cognitive and behavioral supports. 

Community Alternative Care (CAC) Waiver

Is a program for chronically ill and medically fragile persons who need the level of care provided in a hospital. 

Community Access for Disability Inclusion (CADI) Waiver

Is a program for adults age 18-64 years who have disabilities and require the level of care provided in a skilled nursing facility. 

Developmental Disabilities (DD) Waiver

Is a waiver for persons with developmental disabilities or related conditions who need the level of care in an Intermediate Care Facility. 

Elderly Waiver (EW)

Is for adults over age 65 years who need the level of care provided in a skilled nursing facility. 

Alternative Waiver (AC)

Is for older adults who require the level of care provided in a nursing facility and are not yet eligible for MA.  

Essential Community Supports (ECS)

Is a community-based service for people who do not meet nursing facility level of care criteria and are not eligible for Medical Assistance.
