Is a program offering certain home and community-based services for Minnesotans at least age 65 years, at-risk of nursing home placement, and with low income and assets.
Is for people with a traumatic or acquired brain injury, and who need the level of care provided in a nursing facility or neurobehavioral hospital. This program offers specialized cognitive and behavioral supports.
Is a program for chronically ill and medically fragile persons who need the level of care provided in a hospital.
Is a program for adults age 18-64 years who have disabilities and require the level of care provided in a skilled nursing facility.
Is a waiver for persons with developmental disabilities or related conditions who need the level of care in an Intermediate Care Facility.
Is for adults over age 65 years who need the level of care provided in a skilled nursing facility.
Is for older adults who require the level of care provided in a nursing facility and are not yet eligible for MA.
Is a community-based service for people who do not meet nursing facility level of care criteria and are not eligible for Medical Assistance.
