For Minnesotans 65+ at risk of nursing home placement, offering home and community-based services.

Brain Injury Waiver

For individuals with traumatic or acquired brain injuries, providing cognitive and behavioral support.

Community Alternative Care (CAC) Waiver

For chronically ill and medically fragile individuals needing hospital-level care.

Community Access for Disability Inclusion (CADI) Waiver

For adults 18–64 with disabilities requiring skilled nursing facility-level care.

Developmental Disabilities (DD) Waiver

For individuals with developmental disabilities or related conditions needing Intermediate Care Facility-level care.

Elderly Waiver (EW)

For adults 65+ needing skilled nursing facility-level care.

Alternative Waiver (AC)

For older adults requiring nursing facility-level care but not yet eligible for MA.

Essential Community Supports (ECS)

Community-based services for individuals who do not meet nursing facility criteria or are ineligible for MA.
